Resource Navigation and Case Management

A comprehensive case management program providing individualized support to connect at-risk community members with essential resources including housing, healthcare, legal services, employment assistance, and benefits enrollment.
Service Model:
  • Client-centered, trauma-informed, culturally responsive approach
  • Individualized service planning with 2-3 primary goals per client
  • Warm handoffs and accompanied referrals to partner organizations
  • Transportation assistance to critical appointments
  • Follow-up support and advocacy within resource systems
  • Documentation through HMIS (Homeless Management Information System)
Impact Metrics:
  • 14 unique clients served so far (in the first 500 days of program launch) with comprehensive case management for all clients
  • Multiple family members connected to services through primary clients
  • Successful connections to housing, healthcare, legal aid, and employment resources
  • Average caseload of 5-7 clients per case manager
  • Services provided in multiple languages to serve diverse immigrant populations
